Transition Home
We are pleased to announce the opening of our ‘New Transition Home’.
Early preparation and careful planning are essential to make certain that the Transition Process is as smooth as possible for the young person. Looking at their current needs and what they will be in future we draw up a transition plan. Where ever possible the young person will be involved. The transition plan takes into account important areas in their lives and their future, such as housing, further education, health an and level of care, hobbies and work opportunities.
Woodlands
We are proud of the achievements over the past 15 years in offering adult placements to young people leaving Progress School and entering into adulthood.
However we recognise the need to offer a transition home from age 16 for some of our young people who need to start the process early. Woodlands home supports the young people to explore the opportunities available whilst living in the same home, with the same carers. This is a way to help the transition to adulthood run as smooth as possible as this is a very difficult time in their lives.
The staff at our school continue to support the young people through the learning process using the ASDAN – Towards Independence Award Scheme. The correct staffing ratio is provided in the home to enable the young people to aquire skills in the home and the community.
We are fortunate that we have strong links with the local college that offers a range of opportunities and they assist us is finding appropriate courses.
The home is within walking distance of a supermarket, leisure centre, doctors, pub and country park. It is on a bus route into the village which has numerous eating places and a cinema. Preston City Centre is a short bus or train journey.
In the evening and at weekends the staff organise a range of leisure activities, supported home visits or a peaceful environment to relax.
